Insertional Peroneal Tendonitis: What to Know

How the structures involved in Tendinopathy differ from normal

Short answer

Insertional peroneal tendonitis is irritation where a peroneal tendon attaches near the outer foot or ankle, often causing activity-related pain and tenderness that need gradual load management.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Typical location
Outer ankle or outer foot near a tendon attachment
Common trigger
Running, jumping, hills, or uneven ground
Rehabilitation principle
Progressive loading matched to symptoms and strength

What insertional pain means

The peroneal tendons run along the outer lower leg and help control the foot and ankle. Insertional symptoms are felt near the attachment to the outer foot, commonly with walking on uneven ground, running, jumping, or pushing off. Tenderness may be local, with stiffness after rest and pain that rises when the tendon is loaded. A recent ankle twist can cause a tear or other injury instead. Outer-foot bone pain, marked bruising, or difficulty bearing weight should not automatically be labelled tendonitis. [Muscle Soreness After Exercise](/symptoms/muscle-soreness-after-exercise/) usually feels more diffuse and settles as ordinary recovery progresses.

How it is usually managed

Reduce running, jumping, hills, and uneven-surface activity that increases symptoms, while keeping comfortable movement. Supportive footwear and a temporary reduction in training load may help. A physiotherapist can assess ankle strength, balance, foot control, and workload, then build a staged programme through [Physiotherapy](/treatments/physiotherapy-and-rehabilitation/physiotherapy/) and [Strength Training Rehabilitation](/treatments/physiotherapy-and-rehabilitation/strength-training-rehabilitation/). Do not rush from rest into full sport; tendon capacity generally needs progressive loading. Shockwave therapy or PRP injection may be discussed in selected persistent cases, but suitability depends on the examination.

When to seek care

Arrange assessment if pain persists despite reducing load, repeatedly returns when activity resumes, or limits normal walking. Seek prompt care after a twist or impact with a pop, bruising, substantial swelling, weakness, or inability to bear weight. Urgent review is appropriate for severe sudden pain, a visible deformity, or rapidly worsening swelling. A clinician may use examination and imaging to distinguish tendon irritation from a tear, fracture, or another ankle problem.

Questions for your doctor

Ask: Is the painful spot at the tendon attachment? Is there evidence of a tear or bone injury? Which activities can I continue? What loading plan should I follow? When should I consider [Shockwave Therapy](/treatments/physiotherapy-and-rehabilitation/shockwave-therapy/) or [PRP Injection](/treatments/non-surgical-orthopedics/prp-injection/), and what alternatives should come first?

Questions people ask

Can you exercise with insertional peroneal tendonitis?

Comfortable low-impact movement may be possible, but running, jumping, and uneven surfaces should be reduced when they increase pain until a graded plan is established.

When might imaging be useful?

Imaging may help when symptoms follow trauma, weight-bearing is difficult, pain persists, or a tear or fracture needs to be distinguished from tendon irritation.
