What should you know about infected olecranon bursitis?

Short answer
Infected olecranon bursitis is an infection in the fluid-filled sac over the elbow that can cause warmth, redness, swelling, pain, and sometimes fever, so prompt assessment is needed.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Location
- Bursa over the back of the elbow
- Typical clues
- Warmth, redness, swelling, tenderness, and possible fever
- Possible test
- Fluid sampling may identify infection
What it means
The olecranon bursa cushions the bony point at the back of the elbow. Repeated pressure, a small break in the skin, or spread from a nearby infection can allow germs into this sac. The elbow may become tender, hot, red, and visibly swollen. Bending the elbow can be uncomfortable, although the joint itself may still move.
A clinician may look for a skin wound, check the elbow and nearby tissues, and use blood tests or imaging when necessary. Joint Aspiration can remove fluid from the swollen area for testing; this helps distinguish infection from non-infectious inflammation.
What to do now
Keep pressure off the elbow and protect any skin break with a clean dressing. Do not squeeze the swelling, puncture it yourself, or share antibiotics. Treatment may include prescribed antibiotics, drainage, and review of the fluid findings. Anti-Inflammatory Medication may help selected non-infectious inflammation, but it does not replace assessment when infection is suspected.
If infection persists, returns, or leaves a troublesome bursa, Bursa Removal may be considered by an orthopaedic team. The choice depends on examination findings, response to treatment, and the condition of the overlying skin.
When to seek care
Seek same-day medical care for a rapidly enlarging swelling, spreading redness, pus, increasing pain, fever, chills, or difficulty moving the elbow. Go to emergency care if you feel faint, confused, severely weak, or develop breathing difficulty. A red streak moving up the arm or a new wound near the swelling also needs urgent review because infection can extend into surrounding tissue.
Questions for your doctor
Ask whether the bursa or elbow joint is involved, whether fluid should be sampled, which treatment is appropriate, and how the dressing should be managed. Ask when you should return, how to reduce pressure on the elbow, and what symptoms would mean the infection is spreading.

Questions people ask
Is every swollen elbow bursa infected?
No. Pressure, injury, or inflammatory conditions can also swell the bursa. Redness, warmth, drainage, or fever make infection more concerning and warrant assessment.
Can I drain an olecranon bursa myself?
No. Puncturing it at home can introduce germs or worsen the infection. Drainage, when needed, should be performed by a trained clinician.
