What the sound may mean

Wheezing is usually a high-pitched sound made as air moves through narrowed small airways. Viral infections, including bronchiolitis, can cause it. Irritation, an allergic reaction, or asthma may also be considered, although a first episode in a baby needs clinical assessment rather than a self-diagnosis. A blocked nose can make breathing sound noisy without being true wheeze. The sound may be more noticeable when the infant breathes out.

What you can do now

Keep the infant upright while awake, offer usual breastfeeds or formula more frequently if feeds are tiring, and clear visible nasal mucus gently. Avoid smoke, strong fragrances, and non-prescribed cough or cold medicines. Note when the sound began, whether it follows a feed or exposure, and whether breathing changes during sleep. A clinician may listen to the chest and decide whether supportive care or treatment such as a nebuliser is appropriate.

When to seek care

Seek emergency help if the baby has blue or grey lips, pauses in breathing, severe pulling in between or below the ribs, grunting, marked drowsiness, or cannot feed because of breathlessness. Get prompt medical advice for a new wheeze, worsening breathing, fewer wet nappies, repeated vomiting, or fever in a very young infant. Wheezing after a possible allergen with facial swelling, hives, or sudden breathing difficulty may indicate anaphylaxis and is an emergency.
