What should you know about infant strep rash?

A streptococcal illness may cause a fine, rough-textured rash along with fever, sore throat, swollen neck glands, reduced feeding, or irritability. Infants may not describe throat pain, and a rash can have many other causes, including [Atopic Dermatitis](/conditions/atopic-dermatitis/), [Contact Dermatitis](/conditions/contact-dermatitis/), [Heat Rash](/conditions/heat-rash/), [Drug Rash](/conditions/drug-eruption/), [Ringworm](/conditions/ringworm/), or [Scabies](/conditions/scabies/). The skin findings do not establish the infection; the clinician will consider the whole illness and decide whether testing is appropriate.

Record the temperature, feeding pattern, wet nappies, energy, and when the rash appeared. Keep the infant comfortable with light clothing and offer usual feeds frequently if they are willing. Avoid leftover antibiotics, sharing medicines, or applying a medicated cream based only on the rash. Wash hands after nose wiping, nappies, or contact with secretions, and avoid close contact with vulnerable people until a clinician explains the likely cause and precautions.

Contact a clinician promptly when a rash occurs with fever, poor feeding, vomiting, unusual irritability, or reduced wet nappies. Seek urgent help if the infant is hard to wake, has breathing difficulty, a stiff neck, a purple or bruise-like rash that does not fade when pressed, repeated vomiting, or signs of dehydration. Rapidly worsening redness, swelling, blisters, or skin peeling also needs urgent assessment.

Ask whether the symptoms fit a streptococcal illness or another infection, whether a throat or other test is suitable, and how to manage feeding and fever safely. Explain any recent antibiotics, medicine exposure, sick contacts, sore throat symptoms in the household, and the timing of the rash.
