What should you know about infant raspy cough?

A raspy or hoarse sound may occur when the throat or voice box is irritated, when mucus changes the sound of a cough, or when the lower airways are inflamed. A blocked nose can make an infant cough during feeds or sleep. Reflux can also irritate the throat and trigger coughing, particularly around feeding. Viral illnesses such as a common cold or bronchiolitis may cause cough with a runny nose, fever, wheezing, or faster breathing. The sound alone cannot identify the cause.

Notice when the cough started, whether it is dry or brings up mucus, and whether it follows feeds, crying, lying flat, or contact with a smoky or fragranced room. A short recording can help a clinician hear the sound, but it should not delay care if your infant is struggling to breathe.

Keep your infant comfortably upright while awake, offer usual breastfeeds or formula feeds in smaller, more frequent amounts if coughing interrupts feeding, and clear a blocked nose with saline drops according to the product instructions. Keep smoke, incense, strong cleaning fumes, and perfume away from the child. Check breathing and wet nappies regularly, and allow rest.

Do not give an infant cough or cold medicine unless a clinician specifically recommends it. Do not use honey for a child under one year. Avoid steam from hot water because it can burn skin and does not treat the underlying cause. Arrange a medical review if the cough is worsening, keeps returning, interferes with feeds or sleep, or comes with fever or noisy breathing.

Seek urgent help now if your infant has blue or grey lips, pauses in breathing, severe chest pulling, grunting, marked drowsiness, or cannot cry, feed, or settle because of breathing difficulty. Get prompt medical advice if breathing is faster than usual, the ribs or skin at the neck pull in with each breath, feeds are much shorter, vomiting prevents fluids, or wet nappies become noticeably less frequent.

A sudden raspy cough after choking, or a cough that began while eating or playing with a small object, needs emergency assessment even if the cough briefly improves. In the United Arab Emirates, call local emergency services for severe breathing trouble. For a milder cough, contact your child’s clinician and describe the sound, duration, temperature, feeding, and breathing pattern.

Ask: Could the sound be coming from the nose, throat, voice box, or chest? What signs should make us seek urgent care? Is the feeding pattern providing enough fluid? Should we use saline, an inhaler, or any other treatment? Could reflux, a viral infection, or an irritant be contributing? Bring a list of medicines, the timing of feeds and cough episodes, and any recording you made.
