What it means

Osteomyelitis can develop when germs reach bone through the bloodstream, a nearby infection, or an injury. In an infant, signs may be less specific than in an older child. A baby may cry when a limb is handled, keep one arm or leg unusually still, or appear generally unwell. Swelling, warmth, or redness near a bone can occur, but their absence does not exclude infection.

A clinician may examine movement, skin changes, circulation, and feeding, then use blood tests and imaging. The team may also take a sample when needed to identify the germ and guide treatment.

What to do now

Arrange urgent medical evaluation if an infant has fever with reduced movement, marked tenderness, unusual sleepiness, or feeding difficulty. Do not force a painful limb to move, massage the area, or give leftover antibiotics. Keep a record of the temperature, feeding, wet nappies, and when the movement change began.

Treatment commonly involves antibiotics selected for the suspected infection and adjusted when test results are available. Some babies need hospital observation, fluids, or drainage of an infected collection. Bone Grafting may be discussed only in selected situations where infection has damaged bone or healing requires reconstruction.

When to seek care

Seek emergency care if the baby is difficult to wake, has breathing difficulty, looks blue or unusually pale, has a seizure, cannot keep fluids down, or has a rapidly worsening swollen or discoloured limb. Immediate assessment is also needed if the baby is not feeding and has fewer wet nappies than usual. These signs can indicate a serious infection or dehydration and should not wait for a routine appointment.
