Infant hoarse cough: what parents should know

A rough, barking, or husky sound can happen when the voice box and nearby airway become inflamed. A Common Cold may cause cough, blocked nose, and throat irritation. Acid Reflux can irritate the throat, especially around feeds or when a baby lies down. Bronchiolitis may add fast breathing, wheezing, poor feeding, or tiredness. Allergic Rhinitis and Asthma are also possible considerations, although an infant needs an examination before these are assumed.

Notice when the cough began, whether it is worse at night or after feeds, and whether your baby is taking their usual amount of milk. A cough sound alone cannot identify its cause.

Keep your baby upright during and briefly after feeds, offer their usual feeds more often if needed, and clear a blocked nose with measures recommended for infants. Keep smoke, vaping aerosol, strong scents, and dusty air away. Do not give cough or cold medicines, honey, or leftover prescriptions unless a clinician specifically advises it for your baby’s age. Arrange a medical review if the cough persists, keeps disturbing feeds or sleep, or is changing.

Seek emergency help if your baby is struggling to breathe, has pauses in breathing, turns blue or grey, becomes unusually floppy, or cannot feed because of breathlessness. Get urgent advice for markedly fewer wet nappies, repeated vomiting, a high temperature in a very young infant, or a cough that started suddenly after choking. A clinician can listen to the chest and assess hydration and oxygenation.
