What should you know about an industrial keloid?

An industrial keloid is an overgrown, firm scar that can form around an industrial ear piercing and extend beyond the original injury. It may feel smooth, tight, itchy, tender, or darker or lighter than nearby skin. A raised bump after piercing can also be irritation, a hypertrophic scar, or infection, so appearance alone cannot confirm a keloid.

An industrial piercing passes through two points of the upper ear, leaving more than one healing site and a bar that can press on the tissue. Repeated friction, pressure, snagging, or early jewellery changes may keep the area irritated. In a keloid, scar tissue continues to build beyond the piercing holes instead of staying within the original wound.

A clinician may ask when the piercing was done, whether the bump is enlarging, and whether there is drainage or increasing pain. They may examine both piercing sites and the skin around the bar. Do not squeeze, pierce, cut, or repeatedly apply strong products to the lump; these actions can worsen inflammation or injure the ear.

Arrange a dermatology assessment if a firm bump is growing, spreading past the piercing holes, or staying raised after the piercing has healed. Until then, keep the area clean with gentle washing, avoid twisting the bar, and reduce pressure from headphones, helmets, hair, and sleeping on that ear. Follow professional advice before removing jewellery, because a healing channel can close around irritation or infection.

Treatment is chosen according to the scar’s size, thickness, symptoms, skin type, and previous response. Options may include steroid injection into the scar, pressure or silicone-based scar care, selected laser treatments, freezing treatment, or surgery combined with another treatment to reduce regrowth. A keloid can return after treatment, so follow-up matters.

Seek prompt medical care if the ear becomes increasingly red, hot, swollen, or painful, or if you notice pus, a bad smell, fever, or redness spreading away from the piercing. These features can point to infection rather than scar tissue, and cartilage infections need timely assessment. Seek urgent help after severe swelling, marked colour change, or an injury that tears the ear.

Book a routine appointment for a lump that keeps enlarging, bleeds, cracks, restricts jewellery movement, or causes persistent itching or tenderness. Take a clear photo and note changes in size or symptoms; this can help the clinician compare its progress.

Ask whether the bump fits a keloid, hypertrophic scar, irritation, or infection; whether the jewellery should stay in place; which treatment is suitable for an ear keloid; how many visits may be needed; and how recurrence will be monitored. Mention any earlier keloids, family tendency to raised scars, allergies, medicines, or treatments that did not help.
