Prostaglandin for induction of labour

Prostaglandin may be given near the cervix as a pessary, gel or tablet, depending on the maternity unit and your circumstances. It helps the cervix soften, thin and begin to open. Cramping, backache, contractions and discharge can happen as the cervix responds. Your team checks your condition and the baby's heart rate before and after treatment. Prostaglandin is not suitable for everyone, including some people with a previous caesarean scar, so the clinician will review your history. Misoprostol is one medicine you may hear about, but only use what your maternity team prescribes.

Tell the team about previous caesarean birth, allergies, bleeding, leaking fluid, contractions and any medicines you take. Ask how long monitoring will last and whether you can walk, eat or drink. If the cervix becomes ready, the next stage may involve breaking the waters or an Oxytocin infusion. If prostaglandin does not achieve enough change, the team can explain a mechanical method or another birth plan. A membrane sweep may be discussed before formal induction.

Tell staff straight away about heavy bleeding, severe or continuous abdominal pain, very frequent contractions, leaking fluid, fever, faintness or reduced baby movements. If you are at home between appointments, contact your maternity unit when symptoms begin or worsen. Do not take an additional dose or insert another product unless the prescribing team tells you to do so.

Ask which form of prostaglandin is proposed, why it suits your situation, how the baby's heart rate will be checked, what side effects need prompt attention and what happens if contractions become too strong. Ask whether Oxytocin or a mechanical option may be used next.
