What should you know about indications for liver biopsy?

A liver biopsy removes a small tissue sample for examination under a microscope. It may help distinguish cancer from a non-cancerous lesion, identify whether a cancer began in the liver or spread from another organ, or clarify inflammation, scarring, and other liver conditions. Imaging such as MRI can show where an abnormality is, but a tissue sample may be needed when the images do not provide a clear answer or when treatment depends on the tumour type.

The sample can be examined with additional laboratory methods. Immunohistochemistry may detect proteins that help classify cells, while genomic tumour profiling may identify features relevant to treatment selection. These tests are not needed for every biopsy.

Before the procedure, tell the team about blood-thinning medicines, bleeding problems, allergies, pregnancy possibility, and liver conditions. Ask whether the biopsy will be guided by ultrasound or another scan, how pain relief will be provided, and whether you need a driver or observation afterward. Follow the instructions about fasting and medicines exactly, because preparation varies with the method and your health.

A biopsy is not always the first step. Your clinician may compare scans, review blood tests, or decide that tissue would not change management. Ask what question the biopsy is intended to answer and how the result could affect the next treatment decision, including whether chemotherapy is being considered.

After a liver biopsy, seek urgent medical help for severe or increasing abdominal or shoulder pain, fainting, marked weakness, heavy bleeding, a swollen abdomen, breathing difficulty, fever with worsening illness, or repeated vomiting. Contact the procedure team for persistent pain, redness, drainage, or any concern about the puncture site. Follow the discharge sheet even if symptoms seem mild.

Ask why tissue is needed, whether another scan could answer the same question, which route will be used, when results are expected, and which laboratory tests will be performed. Confirm how to restart medicines, what activity to avoid, and who will explain the report.
