Implantation bleeding after sex: what to know

Bleeding after sex is called postcoital bleeding. Friction, vaginal dryness, a sensitive cervix, or a small surface injury can cause it. Implantation, when it occurs, is attachment of an embryo to the uterine lining; the timing of sex and bleeding cannot establish that process.

Bleeding after sex can also occur with chlamydia, cervical dysplasia, cervical cancer, vaginal cancer, or genitourinary syndrome of menopause. Most episodes have a non-cancer explanation, but the symptom should be assessed when it repeats, persists, or has no clear trigger.

Keep a record of when bleeding occurs, its amount and duration, and any pain, discharge, itching, urinary symptoms, or missed period. Pause penetrative sex if it causes pain or bleeding, and address dryness with a suitable lubricant. Arrange a clinical review if the symptom recurs or you may be pregnant.

A clinician may check the vagina and cervix, offer pregnancy or infection testing, and review screening history. A cone biopsy may be discussed only if examination or screening identifies a cervical area that needs tissue assessment or treatment.

Seek urgent care for heavy bleeding, severe pelvic pain, fainting, severe dizziness, fever, or a possible pregnancy with pain or bleeding. Book a prompt appointment for recurrent post-sex bleeding, bleeding after menopause, unusual discharge, or pain during sex.
