Iliotibial Band Bursa: What to Know

Short answer
An iliotibial band bursa can become irritated where the band moves near the outer hip or knee, causing local tenderness, pain, and sometimes swelling.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Typical area
- Outer hip or outer knee
- Common triggers
- Repetitive bending, running, stairs, or side pressure
- Red-flag change
- Rapid swelling with warmth, redness, or fever
What a bursa does
A bursa is a small fluid-filled sac that helps tissues glide over one another. Repeated friction, pressure, a sudden change in activity, or altered movement can irritate it. Pain is usually felt on the outside of the hip or knee and may worsen with stairs, running, lying on the affected side, or repeated bending. Iliotibial band pain, tendon problems, joint disease, and referred pain can feel similar, so an examination helps locate the actual source. Swelling may be subtle or more obvious around the involved area.
Ways to settle irritation
Reduce the activity that repeatedly rubs or compresses the painful area, then maintain comfortable movement rather than complete inactivity. A clinician may recommend a gradual strengthening plan for the hip and leg, technique changes, and short-term symptom relief. Anti-inflammatory medication may be considered when appropriate for your health history; ask a pharmacist or clinician about interactions and precautions. Do not place ice directly on skin or use medication that has not been checked for you.
When to get assessed
Book an assessment when pain affects sleep, walking, work, or exercise, or when symptoms continue despite reducing the trigger. Seek prompt medical care for rapidly increasing swelling, redness or warmth, fever, a wound near the area, severe pain after an injury, or inability to use the joint. If the diagnosis remains uncertain, a clinician may consider imaging or joint aspiration to distinguish inflammation, infection, blood, or another cause.
What to ask
Ask whether the painful structure is the bursa, iliotibial band, tendon, or joint; which activities to modify; and which exercises are safe. Discuss medication risks, when an injection might be considered, and what changes would require reassessment.

Questions people ask
Is every outer-hip pain caused by a bursa?
No. Tendons, the iliotibial band, the hip joint, and referred pain can produce similar symptoms.
Can a swollen bursa be drained?
Joint aspiration may be considered in selected cases to clarify or treat fluid-related problems, but the decision depends on the site and suspected cause.
