Interstitial cystitis and overactive bladder: what should you know?

Short answer

Interstitial cystitis and overactive bladder can overlap, but bladder pain, urgency, and frequency need assessment because their causes and treatments may differ.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Pain matters
Pain or pressure changes the assessment of urinary urgency and frequency.

Note drinks, urgency, pain, leaking, and relief after urination.

Understanding the symptoms

Interstitial cystitis, also called bladder pain syndrome, is associated with bladder or pelvic pain, pressure, and urinary frequency. Overactive bladder is defined by a sudden urge to urinate, often with frequent daytime or night-time visits; some people experience leaking urine. Pain that increases as the bladder fills and eases after urination can point toward a pain-focused bladder problem, but symptoms vary.

A urine test may help check for infection, while a clinician considers pelvic-floor tension, medicines, constipation, and how completely the bladder empties. One diagnosis should not be assumed from urgency alone.

What may help

Keep a bladder diary showing drinks, toilet visits, urgency, pain, leaking, and possible triggers. Caffeine, alcohol, fizzy drinks, acidic foods, constipation, and prolonged holding may aggravate symptoms for some people; identify personal patterns rather than removing many foods at once.

Use fluids sensibly and avoid repeatedly delaying urination until pain is severe. Bladder training may help urgency when a clinician considers it suitable. Pelvic-floor therapy can be useful when muscles are overactive or weak, but exercises should be tailored. Urodynamic testing may be discussed when the diagnosis or bladder function remains uncertain.

When to seek care

Arrange review for ongoing urgency, frequency, bladder pain, or leaking urine, especially when symptoms affect sleep, work, intimacy, or movement. Seek prompt care for fever, pain in the back or side, visible blood in urine, inability to pass urine, vomiting, or rapidly worsening pain.

Questions for your doctor

How will you distinguish infection, bladder pain syndrome, and overactive bladder? Should I keep a diary or have urine testing? Would bladder training, pelvic-floor therapy, or urodynamic testing fit my symptoms? What should prompt urgent review?

Questions people ask

Are interstitial cystitis and overactive bladder the same?

No. They can share urgency and frequency, but interstitial cystitis commonly centres on bladder or pelvic pain, while overactive bladder centres on sudden urgency.

Can overactive bladder cause bladder pain?

Urgency can feel uncomfortable, but persistent or significant pain needs assessment for other causes, including infection or bladder pain syndrome.
