A blood-like smell needs context

Smelling blood when you cough can come from irritated nose or airways, but visible blood or breathing symptoms need prompt medical assessment. A metallic smell may occur when a small amount of blood from a dry nose, gums, throat, or inflamed airway reaches the back of the mouth. Sometimes the smell is noticed without visible blood, and the source may be nasal rather than lung-related. Do not assume the smell alone identifies the cause.

Possible sources

A recent cold, allergies, forceful coughing, dry air, or nosebleed can irritate delicate lining. Gum disease or a mouth injury can also leave a metallic taste or smell. Conditions affecting the airways, including bronchitis, asthma, or bronchiectasis, may produce mucus and sometimes blood. Acid reflux can irritate the throat and create unpleasant sensations, but it does not explain every episode. Note whether you see red or rusty mucus, where it seems to originate, and whether the symptom follows nasal congestion or a coughing bout.

What to do now

If the smell is brief and there is no visible blood or breathing difficulty, arrange a routine medical review if it returns or persists. Avoid smoking and inhaled irritants, sip fluids, and use gentle nasal care if dryness is present. Do not start antibiotics or stop prescribed inhalers based only on the smell. If you cough up material, note its colour and amount for the clinician; avoid swallowing it when possible so its source is easier to describe.

When it is urgent

Seek emergency help for more than a trace of blood, repeated blood-stained mucus, breathlessness, chest pain, fainting, confusion, or blue lips. Prompt assessment is also needed with fever and worsening cough, unexplained weight loss, or a history of lung disease or blood-thinning medicine. A nosebleed that will not stop with firm pressure needs urgent care.
