I have flat feet and my arch hurts: what should I know?

Short answer
Flat feet with arch pain can reflect overload of the foot's supporting tissues, so reduce aggravating activity and arrange an assessment if pain persists or affects walking.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Foot shape
- Flat feet may be painless; pain changes the question
- First adjustment
- Use comfortable supportive footwear and reduce aggravating load
- Assessment focus
- Foot, ankle, walking pattern and nearby tissues
What the combination can mean
Flat feet describe a lower or less visible arch when standing. Some people have this shape without symptoms; pain suggests that the foot, ankle or nearby tissues are being stressed. The discomfort may be related to prolonged standing, a sudden increase in walking, unsupportive footwear, or reduced strength and control through the foot and leg.
Arch pain can also occur with [Plantar Fasciitis](/conditions/plantar-fasciitis/) or another problem, so the foot shape alone does not establish the cause. A clinician can assess both feet, walking pattern, ankle movement and tenderness.
What to do now
Temporarily reduce activities that make the pain sharper, and choose comfortable shoes with adequate room and support. Gentle calf and foot exercises may help when they do not reproduce significant pain. [Physiotherapy](/treatments/physiotherapy-and-rehabilitation/physiotherapy/) can identify movement or strength factors, while [Exercise Therapy](/treatments/physiotherapy-and-rehabilitation/exercise-therapy/) may build tolerance in a gradual way.
[Custom Orthotics](/treatments/non-surgical-orthopedics/custom-orthotics/) are sometimes considered when an examination shows that added support may improve comfort or function. They should fit into a broader plan rather than be used to ignore worsening pain.
When to seek care
Arrange an assessment when arch or foot pain persists, repeatedly returns, causes a limp, or limits work, exercise or daily walking. Seek prompt care after an injury, or if there is sudden swelling, marked weakness, numbness, colour change, fever or inability to bear weight.
Questions for your doctor
Ask what structure is painful, whether your footwear or walking pattern contributes, and which exercises are suitable. You can also ask whether orthotics are appropriate and how to increase walking or sport without provoking symptoms.

Questions people ask
Do flat feet always need treatment?
No. Treatment is usually guided by symptoms and function rather than foot shape alone.
Can orthotics help arch pain?
They may improve comfort for some people when matched to examination findings, footwear and a wider rehabilitation plan.
