Hysterectomy: do you still have periods?

A total hysterectomy removes the uterus and cervix; a subtotal procedure leaves the cervix in place. Neither type leaves a uterus lining that can shed as a period. If the ovaries remain, they may continue releasing hormones, so you can still notice cycle-related changes such as breast tenderness or mood shifts without menstrual blood. Removing the ovaries causes surgical menopause and can bring hot flushes, sleep changes and vaginal dryness sooner. Hysterectomy may be offered for problems such as Uterine Fibroids, Adenomyosis or Endometriosis, but it is a major decision because pregnancy is no longer possible afterward.

Ask which organs are planned for removal and why. Discuss alternatives, fertility plans, ovarian conservation, the surgical route and expected recovery. Review current medicines, smoking, allergies and any history of blood clots with the surgical team. Light vaginal discharge can occur during healing; follow the discharge instructions about activity, bathing and sex. Keep a record of any bleeding, pain or fever so the team can assess changes clearly.

Contact your surgeon promptly for bleeding that is heavier than light spotting, worsening pelvic pain, fever, foul-smelling discharge, wound redness or difficulty passing urine. Seek urgent help for severe pain, fainting, sudden breathlessness or chest pain. Bleeding months after surgery is not a menstrual period and should be assessed, particularly if the cervix remains.
