Hypothyroidism and spotting between periods

Low thyroid hormone can affect the communication between the brain, thyroid, and reproductive system. Periods may become irregular, heavier, or less predictable. Spotting may instead relate to contraception, pregnancy, infection, polyps, fibroids, medication effects, or changes around menopause. A bleeding pattern cannot diagnose hypothyroidism on its own.

A clinician may ask about the timing, colour, amount, pregnancy possibility, contraception, pain, and other thyroid features such as fatigue, constipation, cold intolerance, or hoarse voice. Testing can help separate thyroid-related changes from other explanations.

Keep a simple record of bleeding dates, amount, pain, missed periods, and medicines. Take a pregnancy test if pregnancy is possible, and arrange a medical review for new, repeated, or unexplained spotting. Do not stop hormonal contraception or thyroid medicine without advice. Mention dizziness, unusual tiredness, or bleeding after sex during the appointment.

Seek urgent care for heavy bleeding with weakness or faintness, severe or one-sided pelvic pain, or bleeding during a possible pregnancy. Prompt medical review is also needed for bleeding after menopause, fever, or bleeding that keeps recurring. Urgency depends on the bleeding pattern and accompanying symptoms, not on whether thyroid disease is common.
