Hypertrophic scar reduction

Reduction can mean making a scar flatter, softer, paler, less itchy, or easier to move across. The scar should be examined to distinguish it from a keloid, an active wound problem, or another skin change. A clinician considers how long it has been present, whether it stays within the original injury, and whether it is still changing.

A dermatologist may offer intralesional steroid injection to soften or flatten selected scars. Fractional laser treatment may be discussed for texture or colour concerns. Scar revision surgery or surgical excision can alter a bothersome scar, though the revised area still needs careful healing and may raise again. Sun protection and avoiding friction help prevent colour contrast and irritation.

Arrange an examination if the area is enlarging quickly, becomes painful without a clear injury, bleeds, breaks down, or develops warmth and drainage. New redness spreading away from the scar, fever, or sudden swelling warrants prompt care. Do not inject, cut, or aggressively abrade scar tissue at home.

Ask which feature should be targeted first: height, firmness, itch, colour, or movement. Discuss realistic changes, the number and spacing of visits, skin-colour considerations, aftercare, and how the plan changes if the scar becomes active again.
