How to diagnose a wrist sprain

Short answer
A wrist sprain is diagnosed through the injury history, an examination of tenderness and movement, and imaging when a fracture or more serious ligament injury cannot be ruled out.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

How clinicians assess it
A clinician asks whether the wrist was bent, twisted, or struck, and what happened immediately afterwards. They check swelling, bruising, alignment, skin condition, grip, finger movement, sensation, and blood flow. Gentle pressure over different wrist bones and ligaments helps locate the injured area; movement and strength are compared within safe limits. Pain after a fall does not identify a sprain by itself, because fractures and tendon injuries can feel similar.
Before and during assessment
Until the wrist is assessed, avoid lifting and repeated gripping, remove rings if swelling is developing, and keep the hand supported in a comfortable position. Do not force a painful range of motion or try to straighten a visibly deformed wrist. Depending on the examination, an X-ray or another scan may be considered. After serious injury is excluded, [Physiotherapy](/treatments/physiotherapy-and-rehabilitation/physiotherapy/) can address motion and strength; [Splinting](/treatments/trauma-and-fracture-care/splinting/) may protect the joint during early recovery.
When to seek care
Seek urgent assessment for deformity, an open wound, rapidly increasing swelling, severe pain, or inability to move the wrist or fingers. Numbness, a cold or blue hand, or loss of finger movement may signal a problem with nerves or circulation. Even without these signs, arrange a review after a fall when pain is focused over a wrist bone or normal use remains difficult.
Questions for your doctor
Ask which structure seems injured, whether imaging is needed now or later, and what findings would change the diagnosis. Clarify how to support the wrist, when to begin movement, and when a follow-up examination should occur.

Questions people ask
Can I diagnose a wrist sprain at home?
You can notice suggestive symptoms, but a home check cannot reliably exclude a fracture or significant ligament injury.
Does every wrist sprain need an X-ray?
Not necessarily; the decision depends on the injury mechanism, examination findings, and concern about a fracture.
