What runners notice
Shin splints, also called medial tibial stress syndrome, usually cause a spread-out tender ache along the inner border of the shin. It may begin during a run or afterward and can become more persistent when training continues without enough recovery. A sudden increase in distance, hills, speed work, hard surfaces or running frequency can increase load on the lower leg. Calf tightness, reduced strength, worn shoes and changes in running mechanics may also contribute.
A clinician checks the location of pain, hopping and walking tolerance, strength, flexibility and training history. A small, very focused area of tenderness, pain at rest or pain that worsens with impact can require assessment for a bone stress injury instead.
Adjusting training

Resume running with easy intervals and build one change at a time. Strength work for the calf, foot and hip may support a return, while a physiotherapist can review technique, surfaces and footwear. Do not run through steadily worsening pain.

Book an assessment if pain is focal, present at rest or night, causes limping, returns whenever running resumes, or does not improve after reducing impact. Seek urgent care for an acute injury with severe pain, marked swelling, numbness, a cold or pale foot, or inability to bear weight.
