Fluid behind the eardrum can remain after the infection settles, causing pressure, popping, or reduced hearing for a while. Antibiotics help only when bacteria are involved; some middle ear infections improve without them, depending on age, severity, and examination findings. Ear pain may need separate pain relief even after the medicine has started addressing infection.

Give or take antibiotics at the prescribed times and complete the course. Use age-appropriate pain relief only as advised, and do not put oil, drops, cotton buds, or other objects into the ear unless prescribed. Keep follow-up if hearing remains reduced, because persistent fluid can affect listening and speech in children. Avoid sharing leftover antibiotics.

Arrange prompt review if pain or fever is worsening, not beginning to settle, or returns after improvement. Seek urgent help for swelling or redness behind the ear, a new facial weakness, severe dizziness, unusual sleepiness, stiff neck, or severe headache. Breathing difficulty or facial swelling after medicine requires emergency care.
