How the treatment choice is made

An ankle fracture can involve one or more bones and may affect the joint surface or surrounding ligaments. Assessment usually considers pain, swelling, skin condition, circulation, nerve function and whether the ankle remains aligned. X-rays and sometimes further imaging help guide the plan.

A stable fracture in a suitable position may be managed with a walking boot or fracture cast, with weight-bearing instructions that change as healing progresses. A displaced or unstable fracture may need the bones realigned and held with fixation surgery. The precise choice depends on the fracture pattern, soft tissues, general health and mobility needs.

Care during recovery

Keep the cast or boot dry and follow the prescribed weight-bearing limit. Elevating the leg can help swelling, while gentle movement of exposed toes and joints may reduce stiffness when allowed. Do not adjust the immobilisation or begin loading early without guidance. Follow-up checks confirm alignment and healing and may lead to a gradual transition out of protection.

Post-operative rehabilitation, or rehabilitation after immobilisation, restores ankle movement, calf strength, balance and walking control. Progress is individual and should follow the treating team’s instructions.

When urgent help is needed

Seek emergency assessment after an injury with visible deformity, an open wound near the fracture, severe pain, or inability to bear weight. Get urgent help if toes become blue, cold, numb or increasingly swollen, or if a cast becomes extremely tight. Fever, worsening redness, drainage or a sudden increase in pain during recovery also needs prompt review.
