How is hip fracture different in older adults?

Short answer

A hip fracture differs in older adults because a relatively minor fall can disrupt mobility and independence, making prompt assessment and coordinated recovery especially important.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Possible cause
A fall or low-impact injury
Main functional effect
Sudden loss of safe walking and transfers
Recovery needs
Medical treatment, mobility support and rehabilitation

Why the impact can be broader

A hip fracture is a break in the upper thigh bone near the hip joint. In an older adult, bone may be more fragile and balance, vision, muscle strength or medication effects may contribute to a fall. Pain may be felt in the hip, groin, thigh or knee, and the person may be unable to stand or put weight through the leg. Some people can still take a few steps, so movement alone does not rule out a fracture.

The injury can interrupt transfers, walking, dressing and other daily tasks. Hospital assessment also considers existing medical conditions, medicines, hydration, skin protection, mobility and the support needed after discharge. Treatment may involve surgery or another stabilising plan, followed by early supervised movement when appropriate.

What to do after a fall

Do not pull the person up or ask them to walk if hip fracture is possible. Keep them warm and still in a comfortable position, avoid food or drink until medical advice is available, and call emergency services for assessment. Tell the team about the fall, pain location, medicines, blood thinners, allergies and usual walking ability. After treatment, follow the mobility plan, use prescribed aids, keep walkways clear and accept help with transfers until strength and balance improve.

When urgent care is needed

Seek emergency care after a fall when there is new hip or groin pain, inability to stand, a leg that appears shortened or turned outward, or severe pain with movement. Urgent help is also needed for confusion, fainting, breathing difficulty, chest pain or a new cold, pale or numb foot. A fracture can coexist with other injuries or medical problems, particularly after a fall.

Questions for your doctor

Ask where the fracture is, whether surgery is recommended, and when weight-bearing and physiotherapy can begin. Discuss pain control, blood-clot prevention, wound care, nutrition, fall prevention, equipment at home and the level of help needed for bathing, stairs and walking.

Questions people ask

Can an older adult walk with a hip fracture?

Some people can still take limited steps, so the ability to walk does not safely exclude a fracture after a fall.

Why is early assessment important?

Assessment identifies the fracture and other fall-related problems, then helps coordinate pain relief, treatment and safe mobility planning.
