How effective is premarital screening for sickle cell disease?

Testing commonly uses blood analysis to look for haemoglobin patterns associated with sickle cell disease and the sickle cell trait. A carrier may be healthy but can pass the gene to a child. If both partners carry a relevant haemoglobin variant, counselling can explain inheritance possibilities and whether further testing is appropriate. A screening result is not the same as a full assessment of symptoms or all genetic conditions.

Arrange screening early enough to allow time for results and follow-up. Take the laboratory report to a clinician or genetic counsellor, especially if either result is abnormal or described as a trait. Ask whether confirmatory testing is needed, whether family testing would clarify an uncertain result, and which reproductive options are available in your setting. Do not interpret a carrier result as a diagnosis of sickle cell disease.

Seek prompt medical care for severe pain, difficulty breathing, marked weakness, fainting, fever, or symptoms that feel urgent, whether or not screening has been completed. Screening is preventive information and does not replace assessment of an acute illness. A clinician can also review persistent fatigue, jaundice, or recurrent pain with appropriate examination and testing.

Ask: Does this result show a trait, disease, or an uncertain pattern? Do both partners need confirmatory testing? Should we meet a genetic counsellor? What choices should we discuss before pregnancy?
