How effective is paediatric physiotherapy for juvenile idiopathic arthritis?

Short answer
Paediatric physiotherapy can help children with juvenile idiopathic arthritis maintain movement, strength, physical function, and participation, but the programme must change with inflammation and symptoms.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

How therapy supports JIA
Juvenile idiopathic arthritis can cause joint inflammation, stiffness, pain, and tiredness. A child may move less to protect a sore joint, which can lead to weakness and difficulty with daily tasks. Physiotherapy uses age-appropriate movement, gentle range-of-motion work, strengthening, posture practice, and activity planning to protect function while respecting the child’s symptoms.
The aim is not to force an inflamed joint. The physiotherapist coordinates with the child’s medical team and adapts the plan during flares, after treatment changes, and as school or sport demands change. Swimming, cycling, or other low-impact activities may be considered when they suit the child and clinician’s advice.
Using the programme safely
Practise the recommended movements regularly, with attention to technique and the child’s response during and after activity. Ask how to distinguish expected muscle effort from a flare pattern. Warmth, pacing, rest breaks, supportive equipment, and classroom adjustments may make participation easier.
Keep the rheumatology and therapy appointments even when symptoms are quiet. Report changes in morning stiffness, swelling, walking, hand use, sleep, or school participation so the programme can be reviewed. Medication decisions belong to the child’s treating clinician; physiotherapy works alongside that care.
When to seek care
Contact the medical team when a joint becomes newly swollen, hot, or much more painful, or when stiffness, fatigue, or reduced function persists. Seek urgent care for severe illness, breathing difficulty, sudden weakness, or an injury that prevents weight-bearing. A child with a painful red eye or vision change needs urgent medical attention because JIA can involve areas outside the joints.
Questions for your doctor
Which joints should therapy focus on now? What exercises are suitable during a flare? How much activity is appropriate for school and sport? Would splints, heat, or equipment help a specific task? Which changes in swelling, stiffness, or eye symptoms should prompt an urgent call?

Questions people ask
Can physiotherapy cure juvenile idiopathic arthritis?
Physiotherapy does not cure the underlying inflammatory condition. It supports movement and daily function while medical treatment addresses inflammation.
Should a child exercise when a joint is swollen?
The activity plan may need to change during swelling. Ask the treating team which gentle movements and activities are appropriate at that time.
