How effective is laminectomy for cervical myelopathy?

Cervical myelopathy occurs when the spinal cord is squeezed in the neck. A laminectomy removes the back part of affected vertebrae to enlarge the space around the cord. Depending on alignment, instability and the source of pressure, it may be combined with fusion or approached from the front instead.

The aim may be to stop worsening balance, hand clumsiness, weakness or altered sensation, and some people also gain function. Long-standing cord injury may not fully reverse. Recovery can continue after the wound heals, with physiotherapy focused on walking, balance, hand use and safe strength. The surgeon judges progress through neurological examination and daily activities, not pain alone.

Tell the team about falls, dropping objects, hand numbness, walking changes and any bowel or bladder symptoms. Discuss whether your neck needs stabilisation and what movements are restricted. After surgery, follow neck, lifting and wound instructions, use prescribed support, and attend rehabilitation. Remove trip hazards at home and arrange help with tasks that require carrying or reaching while balance and strength are recovering.

Report a change in walking, hand function or strength at follow-up rather than waiting for the next routine visit. Do not drive until your surgeon confirms that neck movement, reaction time and medication use make it appropriate.

Seek urgent assessment for rapidly worsening weakness, new loss of balance, loss of bladder or bowel control, severe neck pain after an injury, or new numbness affecting the limbs. After surgery, contact the team for wound drainage, fever, swallowing or breathing difficulty, or a sudden change in arm or leg function.

Is the spinal cord compressed from behind, in front, or both, and will laminectomy alone provide enough space? Is my neck stable and might fusion be needed?
