How effective is induction of labour for obstetric cholestasis?

Obstetric cholestasis, also called intrahepatic cholestasis of pregnancy, affects bile flow and can cause itching without a visible rash. The maternity team uses symptoms, blood tests such as bile-acid and liver tests, pregnancy stage, and fetal assessment to discuss when birth should occur.

Induction starts labour with medicines or procedures rather than waiting for contractions to begin on their own. It may reduce the time spent continuing a pregnancy where cholestasis raises concern, but it cannot remove the condition immediately and does not predict exactly how labour will progress. Some inductions lead to vaginal birth; others may require a different birth plan if labour does not progress or another concern develops.

Keep your maternity appointments and ask how often your bile-acid and liver tests should be repeated. Take prescribed treatment as directed and report a clear change in itching, reduced fetal movement, or new symptoms. Your clinician may discuss a planned induction date, monitoring before birth, and what will happen if your cervix is not ready.

Contact your maternity unit urgently if your baby is moving less than usual, you have vaginal bleeding, your waters break, or you develop severe abdominal pain. Seek immediate assessment for a severe headache, vision changes, sudden swelling, or feeling very unwell, because these symptoms need prompt review during pregnancy.

Ask what your latest bile-acid result means, why induction is being recommended now, how your cervix will be prepared, and which fetal monitoring will be used. Also ask what alternatives are appropriate if labour does not start or progress and when to come to hospital.
