How gait changes may help

IT band syndrome causes pain on the outer side of the knee, often during repeated running or downhill activity. A clinician may watch your walking or running to identify excessive crossover, poor control of the pelvis, or a stride that repeatedly loads the irritated area.

Retraining may adjust cadence, step width, foot placement, posture, or hill and speed choices. These changes aim to spread load more comfortably; they do not directly repair every possible cause of outer knee pain. Hip and trunk strength, footwear, recovery, and training volume may need attention as well.

Using retraining safely

Start with the smallest change your physiotherapist recommends and practise it at a pace that keeps symptoms settled. Record which surfaces, speeds, and distances provoke pain, then increase only one training demand at a time.

Strength work for the hip and leg can improve control alongside gait practice. Temporarily replacing painful hills or long runs with lower-impact exercise may maintain fitness while the knee calms. A gradual return is more useful than testing the knee with a sudden hard session.

When to get reviewed

Arrange a review if pain persists despite reduced loading, appears during ordinary walking, causes limping, or is accompanied by swelling, locking, giving way, fever, or pain after a significant injury. Sudden inability to use the leg needs urgent assessment.
