How effective is fracture casting for growth plate fracture?

A growth plate is an area of developing tissue near the end of a child’s or teenager’s bone. A cast can limit movement and protect the injured area while it heals. The right treatment depends on which part of the growth plate is involved, whether the bone is shifted, and whether the joint is affected.

Because growth can continue after the cast comes off, the clinician may arrange follow-up examinations or images beyond the initial healing period. These checks look for maintained alignment and signs that the bone is developing as expected. Do not try to straighten the limb or remove the cast at home.

Keep the cast clean and dry, elevate the limb if it is swollen, and encourage movement of uncovered fingers or toes only as instructed. Follow restrictions on running, sports and weight bearing. Children may need extra help protecting the cast at school and during bathing. Keep every review appointment, even when pain improves.

Get urgent care for increasing pain or swelling, numbness, colour change, cold fingers or toes, or difficulty moving them. Contact the treating team if the cast becomes wet, cracked, loose or tight, or if it causes a sore area. A new change in limb shape or movement needs prompt review.

Ask which growth plate zone is injured, whether alignment is stable, how long activity restrictions may last, and what follow-up is needed to assess growth. Ask what symptoms mean the cast needs an earlier check.
