Short answer

Exercise therapy can improve strength, foot control, balance, and comfort for some people with symptomatic flat feet, but it does not rebuild an arch in every case.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Main aim
Improve strength, control, balance, and load tolerance.
Arch shape
May not change even when symptoms and function improve.

What this means

Flat feet describe a lower or less visible arch, and some people have no pain or activity limits. When symptoms are present, exercises may train the small muscles of the foot, the calf, and the muscles that guide the ankle and leg. Better control can reduce strain during standing, walking, or sport, although the shape of the foot may remain similar.

A physiotherapist may assess footwear, ankle movement, calf flexibility, balance, walking pattern, and whether pain comes from the foot, ankle, knee, or hip. The plan is adjusted to your findings rather than based only on the appearance of the arch.

How to use exercise therapy

Begin with movements that are comfortable and controlled, such as short-foot exercises, toe control, calf raises, calf stretching, and single-leg balance when appropriate. Progression may add resistance, uneven-surface control, or functional tasks that resemble your daily activities. A gradual increase in effort is usually more useful than forcing a painful range.

Practise the programme consistently and record which activities increase symptoms. Supportive footwear or an orthotic may be considered alongside exercise when alignment or load tolerance needs extra help. Do not change several parts of your routine at once, because that makes it harder to identify what helps.

When to seek care

Arrange an assessment if foot pain persists, one foot changes shape, walking becomes difficult, or symptoms interfere with work or sport. Seek prompt medical care after a significant injury, or if swelling, marked weakness, numbness, skin colour change, or inability to bear weight appears.

Questions for your doctor or physiotherapist

Ask which muscles and movements need attention, how to recognise useful effort versus overload, and when the programme should be progressed. You can also ask whether footwear, an orthotic, or evaluation of the ankle, knee, or hip should accompany the exercises.
