How effective is an ECG for heart failure?

An ECG shows the timing and pattern of the heart's electrical activity. It may identify an irregular rhythm, previous damage, conduction problems or strain patterns that help explain symptoms such as breathlessness, tiredness or ankle swelling. It can also identify rhythms that need separate treatment.

Heart failure is about how the heart fills or pumps and how this affects the body. An ECG does not directly show pumping strength or fluid in the lungs. Clinicians usually combine it with an examination, blood tests and heart imaging such as an echocardiogram to clarify the diagnosis and guide care.

Describe when breathlessness occurs, whether you wake short of breath, and whether swelling changes during the day. Share your medicines, including non-prescription products, and mention previous heart disease, high blood pressure, kidney disease or rhythm problems.

Attend follow-up tests even if the ECG is described as reassuring. A result can be helpful without answering every question about symptoms. Keep a record of changes in weight, swelling and exercise ability if your clinician asks you to monitor them, and take prescribed treatment only as directed.

Seek emergency care for severe or rapidly worsening breathlessness, chest pain, fainting, blue or grey lips, new confusion, or coughing up pink frothy fluid. Sit upright while waiting if that is more comfortable and follow emergency instructions.

Contact a clinician promptly if swelling increases, breathing becomes harder when lying down, or your usual activity suddenly becomes difficult. These changes need assessment even when an earlier ECG did not show a major abnormality.
