How does vaginal pessary fitting work?

A clinician usually asks about pressure, urinary or bowel symptoms, activity and whether you hope to have sex with the pessary in place. They examine the vagina and pelvic support, then try a device chosen for its shape and size. A silicone ring pessary is one possible option, although other designs may be considered when support needs are different. The pessary should support the vaginal walls or uterus without pressing sharply on tissue.

You may walk, sit, cough or pass urine during the appointment so the fit can be assessed in everyday positions. The clinician may remove and replace it, change the size or choose another design. A good fit should feel secure but not painful, and it should not cause persistent bleeding, discharge with a bad smell or difficulty emptying the bladder.

Follow the removal, cleaning and review instructions you are given. Some people return for a check after using the pessary for a period of time; others learn to remove and replace it themselves. Keep follow-up appointments so the vaginal tissue can be examined. Mention dryness or friction, because a clinician may suggest a suitable lubricant or local treatment. Do not force a device back in if it is painful or will not sit comfortably.

Contact the fitting service if the pessary falls out repeatedly, causes pain, rubs, produces new bleeding or makes urination difficult. Seek prompt care for heavy bleeding, fever, severe pelvic pain, inability to pass urine or foul-smelling discharge. A vaginal bulge that suddenly becomes painful or cannot be pushed back needs urgent assessment.
