How does joint fusion surgery work?

Joint fusion surgery removes damaged joint surfaces, positions the bones together, and holds them with fixation until they heal into one stable unit.

What happens during fusion

The surgeon reaches the joint through an incision, clears worn cartilage and prepares the bone surfaces. The bones are aligned in the position that gives the joint useful stability, then secured with screws, a plate, or another fixation method. Over time, new bone can bridge the prepared surfaces. Because the joint no longer moves as a joint, pain from movement may lessen, although nearby joints may take on more work.

Fusion can be considered for selected problems such as severe arthritis in the ankle, big toe, hand, or thumb. It is different from joint replacement, which keeps a manufactured joint surface moving.

Preparing and recovering

Before surgery, discuss medicines, smoking, diabetes, walking aids, and the amount of weight you may place through the operated limb. Afterward, protect the fusion while it heals. This may mean a splint, cast, boot, or restricted weight-bearing, followed by gradual movement and strengthening guided by your surgical and therapy team. Keep the wound clean and attend imaging or review appointments so alignment and healing can be checked.

When to seek care

Contact your care team promptly for increasing wound redness, drainage, fever, worsening pain, new numbness, or toes or fingers that become unusually pale, blue, cold, or difficult to move. Seek urgent help for sudden breathlessness, chest pain, or marked swelling and pain in a calf.
