How does ankle replacement work?

Short answer
Ankle replacement removes damaged joint surfaces and places metal and plastic implant components so the ankle can move with less painful bone-on-bone contact.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Implant role
- Separates damaged joint surfaces while preserving guided movement
- Early recovery
- Protection, swelling control and carefully limited weight-bearing
What happens in the operation
The surgeon reaches the ankle through an incision, removes the worn ends of the shin and ankle bones, and shapes the surfaces to receive the implant. A metal component is fixed to each prepared bone, with a mobile plastic bearing between them. The design aims to preserve controlled up-and-down ankle movement while maintaining alignment.
Ankle osteoarthritis is one reason this operation may be considered, particularly when pain and stiffness interfere with walking despite non-surgical care. Suitability depends on bone quality, alignment, activity needs, soft-tissue condition and previous procedures. A titanium implant may be used as part of the replacement system.
Recovery after replacement
At first, the ankle is protected while the incision heals, and walking may require crutches or another aid. Rehabilitation then works on swelling control, ankle motion, calf strength, balance and a gradual return to walking. Keep the wound dry as instructed and follow weight-bearing limits closely; doing more too soon can irritate healing tissues.
The replacement is intended to support movement, but it does not make the ankle suitable for every high-impact activity. Your surgeon and physiotherapist can tailor footwear, exercises and activity progression to the implant and your daily demands.
When to seek help
Contact the care team for increasing wound redness, drainage, fever, worsening pain, new calf swelling or pain, or a sudden change in the ability to bear weight. Seek urgent help for chest pain or difficulty breathing. A new injury, marked deformity or sudden loss of ankle movement should be assessed promptly.
Questions to discuss
Ask why replacement is suitable for your ankle, what implant materials and fixation are planned, and whether joint fusion surgery is an alternative. Discuss expected movement, walking aids, driving, work, exercise limits, wound care and the signs of infection or implant problems.

Questions people ask
Does ankle replacement remove all ankle pain?
The aim is to reduce pain from damaged joint surfaces, but discomfort can persist during healing and outcomes vary between people.
How is replacement different from fusion?
Replacement uses implant components to retain ankle movement, while fusion joins the bones to create a stable but less mobile joint.
