How common is skin ulcer in pyoderma gangrenosum?

Pyoderma gangrenosum is an inflammatory skin condition in which a painful bump, pustule, or small injury can develop into an ulcer. The sore may have a ragged or undermined blue-purple edge and a moist or draining base. It can enlarge quickly and may be much more painful than its early appearance suggests.

Several other problems can cause skin ulcers, including infection, pressure, circulation problems, and injury. Diagnosis relies on the history and examination, with tests used to rule out other causes. Manipulation or an operation can sometimes worsen an inflammatory ulcer, so the diagnosis should guide wound care.

Contact a dermatologist or wound-care clinician promptly if pyoderma gangrenosum is suspected. Keep the wound covered with the dressing recommended for you, protect the surrounding skin, and do not pick, scrape, or use over-the-counter acids on it. Tell clinicians about inflammatory bowel disease, inflammatory arthritis, or blood disorders, because these can occur alongside the condition.

Treatment often addresses inflammation as well as wound pain and drainage. Do not arrange surgical removal of the ulcer unless the treating team specifically recommends it after reviewing the diagnosis.
