How common are painful periods with uterine fibroids?

Fibroids are growths in the muscle of the uterus. Their position, size, and number can influence symptoms. A fibroid that bulges into the uterine cavity may contribute to stronger cramping or heavier bleeding, while one in the outer wall may cause pressure with less change to the period itself.

Pain can begin before bleeding, intensify during the first part of a period, or continue as a dull ache in the lower abdomen or back. Arrange a medical assessment if cramps repeatedly interfere with daily activities or are becoming harder to control. Treatment may focus on pain, heavy bleeding, the fibroids themselves, or more than one of these. The appropriate option depends on symptoms, fibroid location, pregnancy plans, and other health factors.

Seek urgent medical help for severe or rapidly worsening pelvic pain, fainting, marked weakness, trouble breathing, or bleeding that feels difficult to manage. Prompt assessment is also needed if pain occurs with a positive pregnancy test or possible pregnancy, because pregnancy-related causes need to be checked quickly.

Contact a clinician soon when pain is new, persistent, associated with fever or unusual discharge, or accompanied by bleeding after sex. These features can have causes other than fibroids and should not be explained away by a previous diagnosis.
