How common is neck stiffness in meningitis?

Meningitis is inflammation of the membranes around the brain and spinal cord. Irritation of these membranes can make bending or moving the neck painful and restricted. The stiffness may develop with a severe headache, fever, light sensitivity, nausea, vomiting, confusion, marked sleepiness, or a rash.

Neck stiffness on its own has many possible explanations, including muscle strain and joint problems. It should not be used to diagnose meningitis by itself. The combination, speed of change, and how unwell a person feels are especially important.

If you are concerned about meningitis, seek urgent medical assessment rather than trying to manage the symptoms at home. Tell the clinician when symptoms began, whether they are worsening, any recent infection or exposure, medicines taken, and whether the person can stay awake, drink fluids, walk normally, and answer questions clearly.

Do not drive yourself if you feel confused, very unwell, faint, or unable to move safely. Have another adult stay with an unwell child or adult while urgent help is arranged.

Get emergency help immediately for neck stiffness with a severe headache, fever, confusion, a seizure, a rash that does not fade under pressure, difficulty waking, weakness, trouble speaking, or rapid deterioration. In babies and young children, unusual irritability, poor feeding, limpness, or a bulging soft spot on the head also warrants urgent assessment.

Ask what tests are needed, whether treatment needs to begin straight away, what changes to report immediately, and what follow-up is appropriate after the acute illness.
