How common is itchy scalp in head lice?

Head lice can trigger itching as the scalp reacts to bites and irritation. Itching may be felt behind the ears or near the nape, although the whole scalp can feel uncomfortable. You may notice scratching, a crawling sensation, or small irritated marks. Some people have little or no itching, especially early on.

An itchy scalp can also result from dandruff, dermatitis, psoriasis, dry skin, or a reaction to a hair product. Confirming lice requires finding a live louse or suitable evidence close to the scalp; itching by itself is not enough.

Inspect the hair in bright light, separating small sections near the scalp and paying attention to behind the ears and the nape. A fine-toothed detection comb on damp, conditioned hair may help. Avoid sharing combs, brushes, hats, pillows, or towels while checking. If live lice are found, follow the directions for an appropriate treatment and repeat or recheck exactly as advised.

People who share close contact or bedding may need checking, but avoid applying insecticide products without a clear reason. A pharmacist or clinician can help distinguish lice from flakes or product residue.

Arrange medical or pharmacy advice if you cannot tell whether lice are present, symptoms continue after correctly following treatment directions, or scratching has caused sores or crusting. Seek prompt care for spreading redness, marked swelling, pus, or fever, which may indicate a secondary skin infection.
