Hot flashes after total hysterectomy: what to know

A total hysterectomy removes the womb and cervix; it does not necessarily remove the ovaries. If both ovaries were removed, or ovarian function changes after surgery, hormone levels can shift quickly and hot flashes, night sweats, sleep disruption, or vaginal dryness may appear. If the ovaries remain, symptoms can still have another cause, including natural hormonal change, medicines, infection, or thyroid problems. The operation itself does not explain every episode.

Tell your surgical or primary-care team when symptoms began and whether the ovaries were removed. Keep a record of sweating, sleep, pain, bleeding, medicines, and temperature. Light layers, a cool room, and water may ease discomfort. Ask about hormone and non-hormonal options only after the clinician reviews the reason for surgery, cancer history, blood-clot history, migraines, and current medicines.

Contact your care team promptly for fever, worsening pelvic pain, foul-smelling discharge, wound concerns, or bleeding after surgery. Seek urgent help for chest pain, severe breathlessness, one-sided leg swelling, fainting, confusion, or a persistent irregular heartbeat. Any new bleeding after a total hysterectomy should be assessed.
