What should you know about horrible pain during ovulation?

Ovulation pain, also called mittelschmerz or mid-cycle pain, can feel like a cramp, stab, ache, or pressure on one side of the lower abdomen. It may happen when an ovary releases an egg and can sometimes be accompanied by light spotting or a change in vaginal discharge. The timing may help, but timing alone cannot confirm the cause.

Pain that is disabling, gets worse, returns frequently, or does not settle may have another explanation, such as endometriosis or an ovarian cyst. A pelvic infection, appendicitis, pregnancy-related emergency, or another condition can also cause lower abdominal pain. A symptom does not identify its cause by itself, so avoid self-diagnosing from the point in your cycle.

Rest, use a heat pack wrapped in cloth on the lower abdomen, and drink fluids if you can. Keep a brief record of when the pain starts, which side it affects, how long it lasts, bleeding or discharge, fever, vomiting, and any effect on walking or daily activities. This pattern can help a clinician decide whether examination, a pregnancy test, urine testing, or imaging is appropriate.

A pharmacist or clinician can advise whether an over-the-counter pain medicine is suitable for you, taking account of pregnancy possibility, allergies, stomach or kidney problems, and other medicines. Do not delay care by repeatedly treating severe pain without finding out why it is happening.

Seek urgent medical help for sudden or severe pain, fainting, marked dizziness, heavy vaginal bleeding, a rigid or very tender abdomen, fever, repeated vomiting, shoulder-tip pain, or pain when pregnancy is possible. These warning signs can indicate a problem that needs prompt treatment.

Arrange a medical appointment if the pain keeps returning, lasts beyond the expected mid-cycle episode, interferes with sex or normal activities, or comes with painful periods, pain with bowel movements, unusual discharge, or difficulty becoming pregnant.
