What should you know about the Hoffman sign and cervical myelopathy?

During the Hoffman test, a clinician gently flicks a fingernail and watches for involuntary movement in the thumb or finger. A response can occur in healthy people and can vary with technique, anxiety, or nervous-system sensitivity. Its significance depends on the rest of the neurological examination. Cervical myelopathy means the spinal cord in the neck is being affected, commonly by narrowing around the cord. Cervical spondylosis is one possible cause, but the test alone does not identify the cause or show how severe any compression is.

Doctors also check walking, balance, hand dexterity, strength, sensation, reflexes, and changes over time. Imaging such as magnetic resonance imaging may be considered when symptoms and examination suggest cord involvement.

Book a medical assessment if a Hoffman sign was found with hand clumsiness, dropping objects, altered sensation, arm weakness, neck pain, or changes in walking. Bring details about when symptoms started and whether they are progressing. Avoid forceful neck manipulation until a spinal clinician has assessed possible cord symptoms. Management depends on the examination and imaging. Some people are monitored or treated conservatively; others may be considered for spinal decompression, such as laminectomy, or stabilisation with cervical fusion when compression and instability warrant it.

Seek urgent care for rapidly worsening weakness, new difficulty walking, repeated falls, loss of hand function, or new loss of bladder or bowel control. Emergency assessment is also appropriate after significant neck trauma when weakness, numbness, or severe neck pain appears. Do not wait for a repeat finger test if neurological function is changing; a clinical examination is more important than the isolated sign.
