Hoarse voice and ear pain: what to know

The throat and ear share nerve pathways, so inflammation in the throat or tonsil area can be felt as pain in an ear even when the ear itself looks normal. A viral infection, nasal congestion, or reflux may irritate the throat and voice box at the same time. Earwax, an ear infection, dental problems, or jaw tension can instead cause genuine ear pain alongside unrelated hoarseness.

Pain on one side, pain when swallowing, muffled hearing, discharge, or a tender neck may help a clinician identify the source. Hoarseness with ear pain does not by itself establish a serious cause.

Rest your voice, drink fluids, and avoid smoke and forceful throat-clearing. Do not put cotton buds, oil, or unprescribed drops into the ear. A pharmacist or clinician can advise on suitable pain relief after considering allergies, pregnancy, stomach or kidney problems, and other medicines. Keep track of whether pain changes with swallowing, chewing, or lying down.

Seek urgent care for breathing difficulty, inability to swallow saliva, rapidly increasing throat swelling, severe dizziness, or swelling behind the ear. Arrange an appointment for severe or one-sided pain, hearing loss, ear discharge, high fever, a neck lump, or hoarseness that persists or worsens. Examination of the ear, throat, mouth, and neck may be needed to locate the source.
