What should you know about hoarse sounding cough?

The rough quality may come from swelling or dryness around the voice box, throat or larger breathing tubes. Common cold symptoms can irritate the upper airway. Acute bronchitis may cause a harsh cough after an infection. Allergic rhinitis can add throat irritation through nasal drainage, while asthma may bring wheeze, tightness or cough with triggers such as exercise or cold air. Reflux is another possible source of voice-box irritation.

Listen for changes rather than trying to diagnose from sound alone. Record whether the cough is dry or productive, whether it is worse at night, and whether fever, wheezing or shortness of breath accompanies it.

Drink fluids, rest your voice and avoid smoke, vaping, dust and strong fragrances. Warm liquids can soothe the throat. Honey is suitable only for people older than one year. Check with a pharmacist before choosing an over-the-counter cough medicine, particularly for a child or someone taking regular treatment.

A clinical review is sensible if the cough is persistent, recurrent or interfering with sleep. Bring a list of medicines and describe any workplace or household irritants.

Call emergency services for severe breathing difficulty, blue or grey lips, confusion, choking, or noisy breathing that is rapidly worsening. Prompt care is warranted for chest pain, coughing blood, a persistent high fever or marked weakness.

Arrange an appointment if the cough does not improve, keeps returning, or is accompanied by ongoing hoarseness, wheeze, weight loss or night sweats. The clinician may examine your throat, listen to your lungs and decide whether testing is needed.

Ask whether the pattern fits upper-airway irritation, reflux, allergy, asthma or infection, and what changes should prompt reassessment. Mention smoking or vaping, allergies, recent illness, medicines and any exposure to dust or fumes.
