Possible causes and patterns
Hives form when inflammatory chemicals widen small blood vessels and affect nearby skin tissue. A trigger may be external, such as heat, cold, pressure, exercise, or contact with an irritant, or internal, such as an infection or medicine reaction. Sometimes no single cause is found. Welts can be raised, change shape, and appear in different places. Itch is common but not required. Deeper swelling may indicate angioedema, especially around the lips, eyelids, hands, or feet.
How to investigate a trigger
Photograph the skin and note the time, duration, location, foods, medicines, illness symptoms, temperature, exercise, pressure, and emotional strain. Keep products simple and avoid rubbing or overheating while the skin settles. Do not remove several foods or stop prescribed medicines without professional guidance. A clinician can decide whether antihistamine treatment is suitable and whether the pattern needs further evaluation.
Warning signs
Call emergency services for trouble breathing, throat tightness, faintness, or swelling of the tongue or mouth. Seek prompt care if the rash is painful, bruises, blisters, peels, stays fixed, or comes with fever or marked illness. Recurrent episodes, persistent swelling, or uncertainty about the diagnosis deserve a medical review even when breathing is normal.
