Hives and hand swelling: what to know

Hives with swollen hands can reflect deeper skin swelling, especially when both symptoms begin during the same episode. The surface welts are often itchy, while hand swelling may feel tight, warm, or uncomfortable. This combination needs attention to symptoms affecting the mouth, throat, or breathing.

Hives are raised areas caused by a temporary skin reaction. Fluid can also collect deeper in the skin, producing angioedema in the hands, lips, eyelids, or feet. Triggers may include infection, a medicine, food, insect sting, heat, pressure, or an immune response. Hand swelling can make rings, watches, or gloves tight, so remove them early if you can do so gently. New one-sided swelling with marked pain needs a separate clinical assessment.

Rest the hands, keep them comfortably cool, and avoid rubbing or squeezing the swollen areas. Remove constricting jewellery and choose loose sleeves. Do not apply a new cream or take an unfamiliar medicine while trying to identify the trigger. A pharmacist or clinician can recommend an oral antihistamine when appropriate and check interactions with your regular medicines. Photograph the hands and welts, and record what happened before the episode.

Call emergency services for tongue, mouth, or throat swelling; trouble swallowing or breathing; wheezing; faintness; confusion; or repeated vomiting. Rapidly spreading swelling after a medicine, food, or sting also warrants immediate assessment. Seek prompt care if hand swelling is severe, painful, discoloured, associated with fever, or does not improve. Recurrent episodes deserve a planned review even when breathing remains normal.
