Hip Adductor Tendinopathy: What to Know

How the structures involved in Tendinopathy differ from normal

Short answer

Hip adductor tendinopathy is usually managed with a gradual load programme, movement changes, and assessment of persistent groin pain.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Structure
Adductor tendon near the inner thigh and pelvis
Typical pattern
Pain with repeated or resisted loading
Core treatment
Gradual strengthening and load adjustment

What the tendon problem means

Adductor tendons connect the inner-thigh muscles to the pelvis. Repeated loading from running, kicking, skating, or direction changes can make the tendon painful and less tolerant of sudden effort. Symptoms often develop gradually and may be felt at the upper inner thigh or pubic-area attachment. Muscle soreness after exercise is a different pattern; compare with [Muscle Soreness After Exercise](/symptoms/muscle-soreness-after-exercise/).

Assessment includes the pain pattern, activity history, hip movement, muscle strength, and resisted adduction. Persistent or atypical symptoms may need further evaluation rather than assuming every groin ache is tendon-related.

Treatment and load management

Temporarily reduce the activities that repeatedly provoke symptoms, especially maximal sprints, hard kicking, and wide-stance stretching. Keep comfortable daily movement and use a rehabilitation plan that begins with manageable contractions, then builds adductor and hip strength, coordination, and tolerance to faster loading.

[Physiotherapy](/treatments/physiotherapy-and-rehabilitation/physiotherapy/) can adjust exercise volume and technique as symptoms change. [Strength Training Rehabilitation](/treatments/physiotherapy-and-rehabilitation/strength-training-rehabilitation/) may be useful when weakness or poor load tolerance is present. A clinician may discuss [Shockwave Therapy](/treatments/physiotherapy-and-rehabilitation/shockwave-therapy/) or [PRP Injection](/treatments/non-surgical-orthopedics/prp-injection/) in selected persistent cases; these are not substitutes for a carefully progressed exercise plan.

When to seek care

Book an assessment when groin pain continues after reducing the provoking load, interferes with sleep or walking, or repeatedly returns with training. Seek prompt care after a sudden tear, substantial bruising, rapid swelling, fever, numbness, or inability to use the leg normally.

Useful questions to ask

Ask which tendon or attachment is tender, what exercise dose is appropriate, how pain should respond over the next day, when running can resume, and whether hip-joint or abdominal causes need consideration. If a procedure is suggested, ask about its purpose, alternatives, preparation, and rehabilitation afterwards.

Questions people ask

Is adductor tendinopathy the same as muscle soreness?

No. Tendon-related pain is often localised and repeatedly provoked by loading, whereas ordinary post-exercise soreness tends to follow unfamiliar effort and settles as recovery occurs.

Should I stop all activity?

Usually, reduce the specific loads that provoke symptoms while maintaining comfortable movement and prescribed rehabilitation.
