High blood pressure and tension headache

A tension-type headache is not a reliable measure of blood pressure, so a high reading and headache need separate assessment. Tension headaches often feel like pressure or a tight band and may relate to muscle tension, stress, poor sleep or prolonged screen use. High blood pressure commonly causes no symptoms, while a sudden severe headache can have other urgent causes.

A headache may occur at the same time as a raised reading without the pressure being its cause. Repeatedly high readings still matter because untreated hypertension can damage blood vessels and raise the risk of Stroke or a transient ischaemic attack. Tell a clinician whether pain started suddenly, followed an injury, changes with position, affects vision, or comes with weakness, confusion, fever or vomiting. Those details are more useful than trying to diagnose the cause from a number alone.

Rest in a quiet room, drink water if you can, and measure blood pressure with a correctly fitted cuff after sitting calmly. Write down the reading, headache pattern, sleep, stress, caffeine and medicines. Use a pain reliever only if it is suitable for you; some anti-inflammatory medicines can affect blood pressure or kidneys. Arrange a review for recurring headaches, repeated high readings or headaches that do not settle.

Seek emergency help for a sudden explosive headache, new weakness or numbness, facial drooping, speech difficulty, confusion, fainting, seizure, severe vision change, chest pain or breathlessness. These can be warning signs of Stroke or another emergency. A headache that follows a head injury, occurs with fever and stiff neck, or is steadily worsening also needs urgent assessment. Do not rely on repeating the blood pressure reading to decide whether these symptoms are serious.
