High blood pressure and shortness of breath

Shortness of breath with high blood pressure needs prompt assessment because heart, lung and other causes can overlap. High blood pressure itself often has no noticeable symptoms, so breathlessness should not automatically be attributed to the reading. The timing, severity and accompanying symptoms help determine the safest next step.

Breathlessness can follow infection, asthma, anxiety, reduced fitness or another lung problem. When it appears with swelling, difficulty lying flat, unusual tiredness or rapid weight change, a clinician may assess for Heart Failure. Chest pressure or discomfort with exertion may point toward Angina. High blood pressure can also coexist with kidney disease, which may contribute to fluid changes. The symptom does not prove any one diagnosis, and a blood pressure number cannot explain it alone.

Stop exertion, sit upright, loosen tight clothing and note when the breathlessness began, what triggered it and whether it improves with rest. Check blood pressure only if you can do so without delaying help. Record chest discomfort, wheeze, fever, cough, ankle swelling and medicines taken. Arrange same-day medical advice for new or worsening breathlessness, even when the reading is not very high. Use prescribed inhalers or heart medicines only as directed.

Call emergency services for severe or rapidly worsening breathlessness, blue or grey lips, inability to speak in full sentences, fainting, chest pressure, sweating with nausea, or new weakness or confusion. These symptoms can occur with serious problems including Heart Failure, Angina or Stroke. Do not drive yourself if you feel faint or severely short of breath. If symptoms are mild but persistent, obtain urgent clinical advice rather than waiting for the next routine blood pressure review.

Ask whether you need an examination, oxygen check, heart tracing, chest assessment or blood tests. Tell the clinician about recent infections, sleeping symptoms, exertion limits, smoking, new medicines and any swelling.
