Can high blood pressure cause shortness of breath?

High blood pressure often causes no noticeable symptoms, so breathlessness should not be automatically attributed to a raised reading. Over time, extra pressure can make the heart work harder. If the heart becomes less able to pump or relax, fluid may build up and breathing may feel difficult, particularly when lying down or during activity.

Other explanations include asthma, chest infection, anaemia, a blood clot, angina, or anxiety. A clinician may ask when the breathing problem began, whether it appears at rest or with exertion, and whether you have chest discomfort, cough, swollen ankles, palpitations, or trouble sleeping flat.

Arrange a medical assessment if breathlessness is new, recurring, or limiting normal activity. Bring a list of medicines and recent blood-pressure readings, including how and when they were taken. Do not stop prescribed blood-pressure treatment because you feel breathless; some medicines and other conditions can also affect breathing, so the cause needs checking.

While waiting for advice, sit upright, avoid strenuous activity, and note triggers and associated symptoms. Assessment may include listening to your chest, checking oxygen level and heart rhythm, and arranging blood tests or scans when appropriate.

Seek emergency help now for severe or rapidly worsening breathlessness, breathing difficulty at rest, blue or grey lips, fainting, confusion, or chest pressure that may spread to the arm, back, neck, or jaw. Sudden breathlessness with weakness on one side, facial drooping, or difficulty speaking may indicate a stroke and also needs emergency care.

If symptoms are milder but appear with ankle swelling, waking at night gasping, or a need to sleep propped up, seek prompt medical advice. These features can occur with heart failure and should be assessed rather than watched at home.

Ask what might be causing the breathlessness, whether your blood-pressure treatment is controlling your readings, and which symptoms should prompt urgent help. You can also ask whether you need checks for heart failure, angina, kidney disease, obesity-related breathing problems, or obstructive sleep apnoea.
