High alanine transaminase in a blood test

A high alanine transaminase level can signal liver-cell irritation, but the result needs context and follow-up. ALT is an enzyme found mainly in liver cells; when those cells are stressed or injured, ALT may enter the bloodstream. One result does not identify the cause on its own.

Clinicians consider recent illness, alcohol, medicines and supplements, weight or metabolic health, viral infections, and strenuous exercise. They compare ALT with other liver tests and your symptoms. Conditions such as glandular fever, dengue fever, coeliac disease, Crohn's Disease, or chronic kidney disease may be relevant in particular clinical contexts, but a raised ALT does not diagnose any one of them. Anaemia, brucellosis, and Chronic Fatigue Syndrome also require their own assessment rather than being inferred from ALT.

Arrange follow-up with the clinician who ordered the test and bring a complete list of medicines, supplements, alcohol intake, recent infections, and exercise. Do not stop prescribed medicine without advice. The clinician may repeat blood work, review other liver markers, examine you, or arrange imaging depending on the overall pattern. Avoid starting a detox or unregulated supplement while the result is being clarified.

Seek urgent medical help for yellowing of the skin or eyes, severe or worsening abdominal pain, repeated vomiting, marked drowsiness, confusion, fainting, or unusual bleeding. Contact your clinician promptly if you develop dark urine, pale stools, fever, or rapidly worsening illness. These symptoms need assessment regardless of how high or low one laboratory result appears.

Ask how far the result is outside the expected range, whether other liver tests are abnormal, and when it should be repeated. Ask whether any medicine, supplement, infection, alcohol exposure, or exercise could affect the reading and what symptoms should prompt faster review.
